Taking a sensitive approach to advance care planning
The provider recognised that conversations about future care and end-of-life wishes could be difficult and ensured they were approached with sensitivity and compassion. Rather than relying on formal discussions, staff tailored conversations to each person's preferences, choosing the most appropriate time, setting and trusted individual to lead them. Leaders reviewed this approach regularly, agreeing to revisit discussions at least every six months, or sooner if needed, to ensure people's wishes remained current. By allowing these conversations to be led by the staff member with the strongest relationship, the service helped people feel comfortable discussing sensitive topics and ensured decisions reflected what mattered most to them.

Supporting people to achieve their final wishes
The service went above and beyond to help people achieve goals that were deeply important to them, even towards the end of their lives. When one person expressed a wish to travel to Canada to see a relative, the registered manager and care manager worked together to make it possible. The care manager personally drove the person to the airport and ensured all arrangements were in place for their care during the flight and on arrival, with the same support provided for their return journey. The person was able to fulfil this important final wish and returned home, passing away peacefully three months later. This compassionate, person-centred approach demonstrated the service's commitment to making meaningful life experiences possible, regardless of the challenges involved.

Delivering high-quality end of life care
The provider had maintained Gold Standards Framework accreditation for a number of years, demonstrating a sustained commitment to delivering high-quality end of life care. This reflected a well-established approach to advance care planning and ensured people received compassionate, coordinated and responsive support as their needs changed. Staff were appropriately trained and spoke confidently about providing end of life care with sensitivity, dignity and reassurance, helping people remain comfortable and supported during this important stage of life. Relatives spoke highly of the care their loved ones received, with one commenting they were “Very thankful for making the end of [person’s] life so comfortable and enjoyable,” reflecting the positive impact of the service's compassionate, person-centred approach.

People were given support to plan for important life changes, so they could make informed decisions about their future, including at the end of their life.
People were supported to understand and make decisions about their future care and support, including those relating to potential medical and psychological needs and their wishes for the end of their life. When people had made decisions about their end of life, staff supported people to make sure these were recorded and where appropriate, shared with professionals and others who might need to know, using tools such as advance care plans.
Some people and their relatives did not want to discuss end of life planning. Where this was the case, the provider documented their decision and established what steps would be taken if there was an emergency. The provider had made plans to revisit the subject in the future.

Respectful, supported, bereavement-aware care
People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life.
At initial assessment advanced decisions were discussed and it was identified if there was a respect form in place. A respect from is a document that details a person’s preferences for care and treatment in an emergency, for example if they are for resuscitation or if they would prefer to not be admitted into hospital.
When the provider supported someone with end-of-life care, there was a plan in place called “A Respectful Request” which detailed their individual preferences for example, who they wanted around them and whether they would like music on.
Staff told us about times when they had supported people as they came towards the end of their life and felt they had made a difference to them at this time. For example, “I supported a person who recently passed away … to remain at home with their wife as this is what they wished to do. I set up a 24-hour package for them and liaised with specialist nurses to source training so staff could support them with their ventilator so they could remain comfortable.”
The provider also had a bereavement care service, who offered free training and support for people and their loved ones. The aim was to ensure those grieving felt supported with compassionate, sensitive and practical care.

People were given exceptional support to plan for important life changes, so they could make informed decisions about their future, including at the end of their life.
People were supported to make decisions about their future care and support, including those relating to potential medical and psychological needs and their wishes for end of their life. If people did not wish to discuss their end of life, clear guidance was in place showing steps for staff to take in an emergency medical situation. Where people had not wished to discuss the end of their lives, the staff team had regularly revisited the subject with a person, to ensure their views were considered.
The care plans we reviewed showed an exceptional level of personalisation, with clear information on how people wished to be supported at the end of their lives. Details were also included on how people wished their lives to be celebrated by others, and what they would like this to involve. Where people had made advanced decisions about their care at the end of their lives, this was clearly recorded in care plans.
Staff we spoke with showed empathy and understanding of caring for people at the end of their lives. A member of staff told us they were passionate about ensuring dignity for people was maintained at the end of their lives. Staff spoke of their sense of loss when a person had recently passed away. The whole staff team felt people were like a family to them, and the care provided for people should reflect this.

Respectful, personalised, end-of-life planning
People were given exceptional support to plan for important life changes, so they could make informed decisions about their future, including at the end of their life.
The service supported people at the end of their lives. The management team explained how they could ensure people received high quality clinical care, due to their dual registration. This allowed people and their relatives to have choices around remaining in the service, without the potential need for a distressing move.
We found people’s advanced decisions and what mattered to them was clearly recorded in their plans of care. Staff supported people and their relatives to create detailed care plans. People had the opportunity to express their wishes around advanced care. We saw people had highly personalised and detailed funeral plans where they had chosen to discuss this.
Staff we spoke with showed empathy and understanding of caring for people at the end of their lives. One staff member we spoke with described this as, “A privilege.” The service worked well with external health and social care teams to ensure people’s wishes and feelings about their palliative care needs were implemented and fully respected. All of the staff team had received training in dying and bereavement and understood the importance of people’s needs and wishes being met.

Driving forward End of life care improvements
There were members of staff with the specific skills to understand and meet the needs of people and their families in relation to emotional support and the practical assistance they needed at the end of the person's life.
The end of life champion was highly motivated, driven, clinically and professionally skilled. They led the team, providing them with support, guidance and training and identified areas for improvement.
This was done through team, relatives and external professionals' feedback and aimed at ensuring each person experienced a compassionate, dignified and individualised end-of-life journey. They cascaded information to the team and led on the development of advanced end-of-life care plans. They delivered bespoke advanced care planning training to senior staff, monitored and audited the developed care plan. This allowed them to identify areas for further development and celebrated successes along the way.

A practical and workload friendly approach to person-centred end of life care
Within Cavendish Professionals 国产乱伦care, with an ever-increasing need for dementia, palliative and end of life services, we actively engaged with the new Lantern Model of nursing care. This model supports palliative and end of life services, especially within a home or care home setting.
What sets this contemporary model apart from others is how it easily includes all staff, despite being nurse-led, and its outcomes all reflect best practice as totally person-centred. This model is exciting and workable in any setting.
The model was very well received by staff and doesn’t increase workload. Rather, it builds on the very good practice already being evidenced, and complements this.
Outstanding care must be totally inclusive of a client’s needs and wishes – this is what the promotes. Our staff had access to the four available webinars of discussion about the Lantern Model – with guest expert speakers who encourage staff with implementing this new method of providing outstanding palliative and end of life provision.
The model also supports families, as it fitted well with our involvement with the dementia-friendly ethos we’re already working with. Enabling open and transparent family communications is key in all successful dementia and end of life care discussions.
As ever in health and social care, it’s not what we think about the delivery of our services, but the client, family and CQC feedback that actually measures our accomplishments – and we’ve only got one chance to get this right at end of life care.
