The service celebrated and recorded people's achievements, recognising progress towards personal goals as an important measure of success. Staff supported people to develop practical independent living skills through personalised, step-by-step approaches. For example, one person learned to use a self-service checkout while shopping, and another developed confidence by independently emptying the bin and completing part of their own laundry. Staff also supported one person, who had previously been unable to shop for groceries, to gradually build confidence by starting with a small local shop before progressing to larger stores over time. As a result, the person was able to shop in a supermarket with staff support, demonstrating increased confidence, independence and everyday living skills.

The provider monitored all people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent, and they fully met both clinical expectations and the expectations of people themselves.
Systems were in place to track, evaluate, and improve the quality of care, with a strong emphasis on achieving meaningful outcomes for individuals. For example, care plans included clearly defined goals tailored to each person’s preferences, health conditions, and lifestyle aspirations. These goals were regularly reviewed in collaboration with the person, their families, and relevant professionals. People and their relatives told us they felt empowered by this process.
The provider used a combination of digital tools, such as real time tracking logs and manual audits to monitor progress against people’s outcomes month to month. This included tracking improvements in people’s mobility, emotional wellbeing, medicine adherence, and social engagement. Staff were highly skilled in identifying changing needs and acted swiftly to reassess and adapt care plans. People’s health and well-being was monitored, and any emerging healthcare needs were addressed as soon as possible.
People were empowered to achieve their goals. The management team had good oversight of staff skills and knowledge and were able to match staff to work with people. People’s support was delivered consistently by staff, in a person-centred way that was specific to their needs. People’s skills, life experience and strengths were discussed with them and those involved in their care, to consider how people’s goals, ambitions and outcomes were planned and achieved. Staff introduced new ideas to people to enhance their quality of life, such as new activities, skills, work and education opportunities. People were actively supported and empowered to develop new skills.

Transforming lives through personalised support
The provider consistently monitored people’s care and treatment to continuously improve it. They always ensured that outcomes were positive and consistent, and they met the expectations of people and their relatives.
People living at the service had been supported to achieve incredibly positive outcomes. People were living with support needs which made it more difficult for them to manage how they were feeling when upset or anxious. Completing daily living tasks and going out into the community had in the past been a very difficult thing for people to do. In previous services, people had not been supported with this and as a result had become isolated and withdrawn. This was no longer the case and as a result people were happier and were living lives in they ways they chose and made sense to them.
Staff had worked with people to identify their favourite past times and how they would like to spend their time. They had then used their knowledge and understanding of people’s individual needs to help them achieve their outcomes.
Staff worked with health professionals to help improve people’s outcomes in relation to their physical and mental health and wellbeing. Advice from professionals was recorded in care plans and monitored by the management team.

They ensured that outcomes were positive and consistent, and that they fully met both clinical expectations and the expectations of people themselves. Staff took part in daily management meetings where any changes in people's health and wellbeing were discussed, recorded and actioned. Relatives confirmed that staff identified and responded promptly to changes in their family members health. Regular care plan reviews enabled the management team to have full oversight of the health and social care needs of all people. People and their relatives were involved as partners in their care, and this was supported by the comments we received.
The provider had a proven track record of people achieving positive outcomes. Feedback from commissioners included numerous success stories; they outlined how people’s quality of life had improved vastly after moving into The Rowans. There was evidence of the service nurturing people, allowing them to grow and follow their hopes and desires. There were examples of people benefitting from improved sleep patterns, building trusting relationships, having reduced medications and joining in activities.

Sourcing appropriate healthcare support to achieve outcomes
People had achieved exceptional outcomes to meet their health needs. For example, Staff worked with a GP and hospital to arrange a joined-up approach so one person could receive two different interventions under one sedation. This resulted in the early detection and treatment of a serious illness.
Staff worked very closely with a range of health professionals to ensure people's good health and wellbeing is maintained. They were exceptional in sourcing appropriate healthcare support for individual people. For example, they recently sourced a specialist learning disability Macmillan nurse.

Learning boards, clinical meetings and other ways to monitor people
Learning boards had been created, for example, for diabetes, displaying the risk factors, possible complications, signs to look for and actions to take. There was a similar learning board on end of life care. These had improved staff knowledge and given them more confidence when supporting people in these areas. Infection control refresher posters provided prompts for staff, acting as constant reminders.
Nurses attended regular clinical meetings where key clinical issues were discussed, such as wound management, weight loss and falls prevention. Action plans were in place to ensure that issues were addressed and reviewed, for example, referrals to dieticians or specialist nurses. The registered manager met daily with heads of departments to ensure key messages about people were shared in a timely way. Daily handover meetings were held so staff had up to date information about the people they were supporting.

Sharing personal experiences of MND with others using the service
One person, who had a medical background, was living with motor neurone disease (MND). They offered to talk with staff in small groups to share their experience of the condition, how it progressed, the problems they faced and treatments. Using Malnutrition Universal Screening Tool (MUST) tools to deliver outstanding care
At Vida Healthcare there is a whole team approach looking to deliver outstanding care in regards to people’s food and drink. The key considerations are to provide person centred care, each individual will have a bespoke care plan pertaining to their individual needs.
Vida Healthcare use the Malnutrition Universal Screening Tool (MUST). MUST’ was specifically developed as a tool that can be used for screening for all adults in all settings, whether or not physical measurements of weight and height are possible. You can read more about MUST .
Vida Healthcare works collaboratively with the dietetics department at Harrogate District Hospital and the local universities, dietetic students have learning opportunities and placements spending time with the Vida team. It is essential to have a multi-disciplinary approach to provide specialist dementia care to ensure person centred care and joined up working in daily practice.

Using Functional Behaviour Profile within dementia care home
Staff also used a Functional Behaviour Profile (FBP) which they found excellent in ascertaining individual’s cognitive score, and in turn comparing and contrasting it with the other people living in the home.
The FBP is a nationally recognised assessment which looks at various cognitive areas such as using tools like a hairbrush or razor, completing tasks such as fastening buttons, decision making process time and support needed, showing enjoyment and understanding simple requests.
This enabled the staff team to not only provide appropriate activities and stimulation but also to 'match' people at similar stages of dementia. The FBP was also discussed in the relatives' yearly reviews and this helped loved ones to understand the speed or direction in which the individual's dementia was progressing. This gave loved ones time to prepare for the next stage and consider future support, particularly in enabling stress-free and enjoyable visits for all involved.
