Overview
This event will involve the collaboration of colleagues from across the Greater Manchester (GM) adult social care and health system. It will give people support to optimise outcomes for people being dishcharged from hospital into an adult social care service, avoid readmissions and improve patient flow. Attendees will gain an understanding of hospital discharge processes in GM and will explore opportunities to make improvements and learn from good practice within the GM system.

- Share what’s working well and learn from good practice across the system
- Understand what isn’t working well, from other’s perspectives
- Take away actions that can be implemented to address challenges
- Develop connections from across the system
